Calcium hydroxylapatite (CaHA) dermal fillers sell for hundreds of dollars per millilitre — the same mineral family that ships in drums, repositioned as an injectable that claims to do more than fill: to “biostimulate” the skin into making its own collagen.

How much evidence carries that claim?

In 2023, a systematic review in Frontiers in Medicine went looking for the mechanism studies behind the marketing. The funnel is the story: “of 2,935 identified citations, 12 studies were included for final analysis.” Within those twelve, “collagen production was reported by nine studies, cell proliferation by four, elastic fibers and/or elastin by four, and three studies on angiogenesis.” And the sentence that should hang over every sales deck in the category: “only seven studies had a control group.”

The signal that survives is real but narrow. The best-documented effect is neocollagenesis — one controlled study measured collagen type III expression rising from 2.38 ± 0.81 to 5.26 ± 1.20 four months after injection (p < 0.0001). Beyond collagen, elastin and some angiogenesis, the reviewers’ verdict is flat: “the majority of the studies had methodological limitations,” and evidence on the wider mechanism claims is limited and inconclusive.

Twelve from 2,935 is worth pausing on. A citation count that large reads as a vast literature, yet after the review applied its criteria, the mechanistic core was twelve papers, seven with controls. That is how an evidence base can feel deep while being shallow — volume of discussion standing in for volume of data. Anyone doing diligence on an ingredient claim eventually learns to run the same funnel: not “how many papers mention this” but “how many measured it, against a control, in something resembling the product as sold.”

Why does a raw-material buyer care about someone else’s syringe?

Three reasons. First, market navigation: CaHA is where hydroxyapatite’s highest margins live, and anyone mapping demand for the mineral should know that this segment’s scientific foundation is twelve papers deep — a market priced like settled science and evidenced like an early one. Second, the asymmetry it reveals: the burden of proof in aesthetics has, historically, been lighter than the marketing implies. A new supplier imagining an impossible evidence bar should recalibrate — the bar the incumbents actually cleared is visible, and it is not high. Third, the caution it teaches: category-wide mechanism claims (“biostimulation”) can outrun the data for years. Borrowing that language for adjacent products — topicals, oral care, supplements — imports a debt of proof the twelve studies cannot service.

None of this says CaHA fillers fail; clinical safety and effectiveness as fillers is a separate, better-fed literature. The review examined the mechanism — the part of the story that commands the premium.

The pattern is the constant across every market this mineral serves: the claim travels first, and the characterisation is asked for later, usually by whoever has the most to lose. The suppliers built for the long term are the ones holding the data before anyone asks.

Sources

Amiri M, Meçani R, Niehot CD, Phillips T, Kolb J, Daughtry H, Muka T. “Skin regeneration-related mechanisms of Calcium Hydroxylapatite (CaHA): a systematic review.” Frontiers in Medicine, 2023;10 (open access) — ncbi.nlm.nih.gov/pmc/articles/PMC10273839.

Calcium hydroxylapatite fillers are the highest-margin destination for this mineral, sold on a “collagen biostimulator” story.
