The other does nothing. Both datasheets were telling the truth.

What does the SBF test actually predict?

Simulated body fluid — a salt solution mimicking blood plasma’s ion concentrations — became the biomaterials field’s standard screen for bone-bonding potential: soak the material, look for an apatite layer. The method is official; as a 2020 review records, “in 2007, the SBF was registered as ISO 23317, which is the standard solution for the in vitro evaluation of the apatite-forming ability of implant materials.”

How well does it work? The review’s central statistic comes from an analysis of 33 published cases: “in the clear majority of the studies (25/33 cases), the SBF immersion test proved successful in predicting the relative in vivo bioactivity of the materials.” Predictive, then — three times out of four. The other eight cases are where purchasing decisions go to die.

Where do the failures come from?

Partly from chemistry the recipe leaves out. The review relays a standing critique: the test suffers from “the absence of proteins, whereas they are known to play an essential role in controlling apatite nucleation,” from the Tris buffer added to hold pH, and from “the absence of any control for the carbonate content of the SBF solutions.” And partly from procedure: “false negative results (i.e., a material may be incorrectly considered non-bioactive) could be due to following non-standard experimental procedures.” Even the ratio of powder to fluid changes outcomes — one cited recommendation fixes it at 1.5 mg/mL for powders, a detail most certificates never mention.

So a genuinely good material can flunk in a sloppy lab, and a mediocre one can pass in a generous protocol. The sceptics’ position, quoted in the review, is that “bioactivity cannot be predicted from observed apatite forming ability simply on immersion in a so-called simulated body fluid.” The review’s own stance is more moderate: a useful predictor — never proof.

The parallel with other qualification tests is exact. A result that can be moved by recipe, ratio or refresh schedule is not a property of the material; it is a property of the experiment. That does not make it worthless — screening tests earn their keep by being cheap and early — but it moves the burden onto documentation. A 25-of-33 hit rate is respectable for a screen and inadequate for a promise, and the difference between those two uses is whether the protocol travels with the claim.

What should a buyer accept?

Treat “SBF-positive” the way you would treat any test result travelling without its method: incomplete. The questions that make it meaningful are mechanical. Which SBF recipe, and per ISO 23317 or a variant? What mass-to-volume ratio? How many days, with the fluid refreshed how often? Characterised by which technique? A supplier who states protocol alongside result is showing you a measurement. One who states only the conclusion is showing you a word — and the eight-in-33 cases are a reminder of how much room that word leaves.

Consistency is a property of methods before it is a property of materials. The suppliers worth keeping know their test as precisely as their powder.

Sources

Baino F, Yamaguchi S. “The Use of Simulated Body Fluid (SBF) for Assessing Materials Bioactivity in the Context of Tissue Engineering: Review and Challenges.” Biomimetics, 2020;5(4):57 (open access) — pmc.ncbi.nlm.nih.gov/articles/PMC7709622.

“Bioactive — forms apatite in simulated body fluid.” The claim sits on an ISO-registered test that a review found predictive in 25 of 33 cases — and capable of failing a good material over a recipe detail.
