Exposed dentin is perforated by tubules; fluid moves through them; the patient feels it. Plug the tubules and the sensation stops. Under a scanning electron microscope you can watch it happen.

It is also where the fluoride-free positioning meets an inconvenient piece of evidence.

Four groups, seven days

In 2016, Kunam, Manimaran, Sampath and Sekar published an in vitro study in the Journal of Conservative Dentistry using nano-hydroxyapatite derived from chicken eggshell powder. Dentin discs from extracted teeth were pretreated with 17% EDTA to open the tubules, then exposed to a test agent for seven days and examined by scanning electron microscopy and confocal laser scanning microscopy.

Four arms: untreated control, 2% sodium fluoride alone, nano-hydroxyapatite alone, and the two together.

Every active agent occluded tubules to some degree. Hydroxyapatite on its own worked. Fluoride on its own worked.

But the specimens treated with the combination of nano-hydroxyapatite and 2% sodium fluoride showed complete dentinal tubular occlusion, with significantly greater depth of penetration than either agent produced alone. The authors’ conclusion is one sentence: “The combination of nHAp and 2% NaF was the most effective in occluding dentinal tubules.”

Depth is the interesting variable

Occlusion at the surface and occlusion in depth are not the same result, and the distinction matters commercially rather than academically.

A surface plug survives until the next acidic drink or the next brushing cycle. Material that has penetrated the tubule has somewhere to retreat to. That is why the depth-of-penetration finding, rather than the presence or absence of occlusion, is the one that predicts whether a consumer still feels the benefit in week six — and week six is when a repeat purchase is decided.

What a brand can honestly say

So the fluoride-free formulation has a real, evidenced sensitivity claim behind it. Hydroxyapatite alone occluded tubules in this study, and that is not a small thing.

What it does not have is this study’s best result. In these four arms the strongest performance belonged to the combination, and a fluoride-free product is, on this evidence, choosing a market position over the maximum measured effect. That is a perfectly legitimate business decision. It stops being legitimate the moment the marketing implies the opposite.

Two limits belong on the record with it. This is in vitro — dentin discs in a laboratory, not mouths, without saliva, a pellicle or a diet. And it is a single study from 2016.

Which leaves the same closing question as always. A tubule-occlusion result is produced by a specific particle size distribution penetrating a specific geometry. If the particle size drifts between lots, the depth of penetration drifts with it — and the claim quietly stops describing the product in the tube.

Sources

Kunam D, Manimaran S, Sampath V, Sekar M. “Evaluation of dentinal tubule occlusion and depth of penetration of nano-hydroxyapatite derived from chicken eggshell powder with and without addition of sodium fluoride: An in vitro study.” Journal of Conservative Dentistry, 2016;19(3):239–44 — pubmed.ncbi.nlm.nih.gov/27217637.

Fluoride-free is the most commercially valuable phrase in hydroxyapatite oral care.
